Requirements
  • Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
  • Accept patients within your geographical area of coverage
  • Plan, prepare and carry out individually designed programs which provide skilled therapy to patients to restore overall function and independence in the home setting.
  • Develop a plan of care/goals in consultation with the physician, family and patient.
  • Timely and accurate documentation of visit
  • Effectively communicating discharge with family members and other team members

Qualifications:

  • Physical Therapist Program Graduate.
  • NY Physical Therapist License or eligible.
  • Evaluates a patient's need for physical therapy and develops an appropriate treatment plan.
  • Provide appropriate care and treatment to patients based on physician care plan, agency policy and professional standards of care.
  • Instructs family members, PTA's, Home Health Aides, and other caregivers in appropriate physical therapy techniques.
  • Certifications Required: CPR/First Aid
